+No immunizations will be given at the office. Patients will be required to receive any needed vaccines at a local pharmacy or health department.
+All plans include a one-time $50.00 sign-up fee. The one-time sign-up fee is waived if the first year is paid in full at the onset of membership.
+ Home visits are available to those with a membership at an additional cost of $100 within a 30-mile radius.
+ We do request a 30-day written notice of cancellation.

+ If a Patient allows their membership to lapse and later desires to rejoin the practice, the Patient shall be accepted on a space-available basis subject to a $150 re-enrollment fee.​
